Declaration of Common-law Relationship - Member form (if applicable)
Spousal Waiver of Death Benefits Prior to Retirement form (if applicable)
Spouse's Waiver of Designated Beneficiary Status - VPB form (if applicable)
Other Documentation Required
Proof of Relationship - refer to the list of acceptable documents for proof of relationship.
Proof of Member Name Change (if applicable) - refer to the list of acceptable documents for proof of member name change.
Proof of Death (if applicable) - refer to the list of acceptable documents for proof of death.
Processing Time
Changes to spousal relationship status are processed approximately five business days of receiving all required documents.
